The Montana Vital Records Ordering System allows individuals to request birth or
death records through the Department of Public Health and Human Services.
You will be required to provide your name, address, and date of birth for electronic identify verification, and appropriate proof of relationship documents (when applicable) when placing an order ($3 fee per transaction). The identity verification fee is non-refundable and applies whether or not the agency is able to fulfill your order. Once you’ve purchased a certificate, it is non-refundable with the exception if cancelled prior to the agency shipping the documents, the shipping fee will be refunded.
Montana Vital Records are available from 1907 to the present.
